FR — To become a client of the Centre and receive primary care:
Given the numerous requests for services that we receive, the WNCHC places a client’s request for primary care services on a waiting list. The WNCHC regularly accepts new clients, respecting the order in which application are submitted.
Persons who wish to become a client of primary care services at WNCHC must meet the following criteria:
- Does not have a family physician;
- Resides in the Municipality of West Nipissing;
- Agrees to receive all his primary care services at WNCHC (i.e. not registering only to get a second opinion);
If you wish to become a client of the Centre, please fill out the registration form and return it in person to our reception desk, by fax to 705-753-5387, by email at info@cscno-wnchc.org, or by mail to the West Nipissing Community Health Centre, 68 Michaud St, P.O.
